Enterprises and institutions registered in the Area.

III. Required Materials

1. Photocopy of the Organizational Code Certificate (original)

2. Photocopy of the Business License (original)

3. Shanghai Statistics Registration Declaration Form

4. In case of replacement and change of license, the former Shanghai Statistics Registration License shall be returned.

IV. Application Process

(I) Complete the Shanghai Statistics Registration Declaration Form and affix the official seal

② Receive it at the handling point

(II) Go to the handling point carrying all required materials

(III) Handle it in the field and receive the Shanghai Statistics Registration License
